Whether Guatemala needs malaria tablets comes down to which departments you visit. The places most travellers stay typically have no transmission, while the lowland departments in the north and east do.
Hepatitis A, hepatitis B, MMR and typhoid are typically recommended for Guatemala. Rabies and malaria tablets depend on the itinerary, and a trip built around Antigua and Lake Atitlán looks different from one that includes Petén. A doctor can tell from your departments whether tablets are needed at all.

Malaria Prevention for Guatemala
Malaria may be present mainly in the departments of Alta Verapaz, Baja Verapaz, Escuintla, Izabal and Petén, with few cases elsewhere. There is typically no transmission in Antigua, Guatemala City or at Lake Atitlán. Almost all cases are the vivax type, which is usually less severe than the falciparum type but can come back months after you get home.
Malaria pills, also called malaria prophylaxis, do not stop you being bitten. They work by killing the parasite in your bloodstream before it has a chance to multiply and make you ill, which is why they are taken daily through the trip and for a period after you leave.
At Singapore Women’s Clinic, we provide malaria pills in addition to travel vaccinations. It is best to consult our doctor 4 to 6 weeks before you fly, so that any tablets can be matched to your route and to medicines you already take. A later appointment is still worth making, as the course itself is usually started only 2 days before you arrive.
Vaccination Certificate Requirements for Entry and Re-Entry
Whether you need a certificate depends on the countries you pass through or come from. Before you travel, check your full route against the current list of countries with a risk of yellow fever transmission on Singapore's Immigration & Checkpoints Authority website.
Entering Guatemala
No certificate is needed if you arrive directly from Singapore, or from any country without yellow fever risk.
It is required from age 1 if you arrive from a country with risk, including airport transits of more than 12 hours there.
Returning to Singapore
Singapore requires a yellow fever certificate if you have recently travelled to a country with yellow fever risk. It follows your route, not your destination, so it can apply even when the country you are visiting has no risk.
- Applies to all travellers, including Singapore residents, of any nationality.
- Includes airport transits of more than 12 hours in an affected country.
- Valid from 10 days after vaccination, then for life.
- Without a valid certificate you may be quarantined for six days under the Infectious Diseases Act, counted from the date you left the affected country.
- Exempt: Children aged one year and below; Travellers with a medical contraindication to the vaccine.
