What Is a Hormone Blood Test?

A hormone blood test measures the levels of different hormones in your blood, providing information about your overall health, reproductive health and metabolism.

At Singapore Women's Clinic, we provide hormone tests for women and men. You can check a single hormone, such as oestrogen or thyroid stimulating hormone (TSH), or several hormones together with a hormone panel test. The blood sample can be taken at any of our clinics or at your home through our Mobile Blood Drawing service.

When Should You Get a Hormone Test?

A hormone test may be recommended when you have symptoms that could be caused by a hormonal imbalance (a hormone level that is too high or too low), as the results can help guide your treatment.

These symptoms can include:

  • Fatigue
  • Weight changes
  • Skin concerns, such as hormonal acne
  • Hair concerns, such as hair loss
  • Irregular periods

If you have chest pain, unexplained weight loss or lethargy (a persistent lack of energy), we recommend a consultation with our doctor before any tests.

A hormone test may not show the underlying cause of these symptoms, and your results need to be interpreted alongside your symptoms and medical history.

Hormone Test for Menopause

A hormone test is not usually needed to diagnose menopause if you are over 45 and have not had a period for 12 months in a row, as menopause is usually diagnosed from your symptoms and menstrual history.

Hormone blood tests may be useful when:

  • Premature ovarian insufficiency (POI) is suspected, in which the ovaries stop working normally before the age of 40
  • You are under 45 and have had a hysterectomy (surgery to remove the womb)
  • Other conditions, such as thyroid disorders or polycystic ovary syndrome (PCOS), need to be ruled out

Hormone levels can rise and fall during perimenopause (the years leading up to menopause), so your results may need to be read alongside your symptoms and menstrual history.

Hormone Test for PCOS

Hormone blood tests can help diagnose polycystic ovary syndrome (PCOS), together with your symptoms and a pelvic ultrasound scan. PCOS may be considered when at least two of the following are present:

  • Irregular or absent ovulation (the release of an egg from the ovary)
  • High levels of androgens (male hormones that all women have in small amounts), seen in your symptoms or blood test results
  • Polycystic ovaries seen on an ultrasound scan

Conditions with similar symptoms, such as thyroid disorders, high prolactin levels (hyperprolactinaemia) and premature ovarian insufficiency, are typically ruled out before PCOS is confirmed.

Types of Hormone Tests

Hormone tests can check a single hormone or a group of hormones in one blood sample.

Female Hormone Tests

Female hormone tests measure oestrogen (estradiol, or E2), progesterone, follicle stimulating hormone (FSH) and luteinising hormone (LH).

Oestrogen and progesterone are made by the ovaries, and their levels decline during menopause. If lower oestrogen levels cause menopause symptoms such as hot flushes, menopausal hormone therapy (MHT) may be prescribed to replace it.

The beta hCG test measures the level of human chorionic gonadotropin (hCG), a pregnancy hormone, in your blood. It can confirm a pregnancy and help monitor how the pregnancy is progressing.

Thyroid Hormone Tests

Thyroid hormone tests check how well your thyroid is working. The thyroid is a small, butterfly-shaped gland at the base of your neck, and its hormones control your metabolism and energy levels.

  • Thyroid stimulating hormone (TSH) – measures how much TSH is in your blood, and is the main test used to diagnose thyroid disorders
  • Free T4 (FT4) – measures the thyroxine (T4) available for your body's tissues to use, rather than the total amount in your blood
  • Free T3 (FT3) – measures the active triiodothyronine (T3) available to your organs and tissues

Testosterone and Other Hormone Tests

The total testosterone test measures the entire amount of testosterone in your blood. In men, testosterone is essential for muscle mass, bone strength and sexual desire.

If low testosterone is found and you have related symptoms, testosterone replacement therapy, a type of hormone replacement therapy (HRT), may be an option. Women also have testosterone in small amounts, and higher androgen levels can be a feature of PCOS.

Other hormone tests include cortisol, insulin and parathyroid hormone (iPTH). Parathyroid hormone helps regulate calcium, phosphate and vitamin D levels in your blood and bones.

Hormone Panel Tests

A hormone panel test checks several hormones in one blood test. Panels are usually grouped around a symptom, such as hormonal acne, weight gain, hair loss or irregular periods, so the hormones linked to it can be checked at the same time. A panel for hormonal acne, for example, may include testosterone, oestrogen, progesterone, cortisol and prolactin.

How Long Does a Hormone Test Take?

A hormone test at our clinic usually takes 30 to 60 minutes during off-peak hours, and your results are usually ready in 7 working days.

We send your results to you by email. You can then review them with our doctor at the clinic or by teleconsultation, so your hormone levels are explained alongside your symptoms and medical history.

How to Prepare for a Hormone Test

Preparing for a hormone test depends on which test you are having, as some hormones need to be checked at a set time of day or at a set point in your menstrual cycle.

  • Oral contraceptives – female hormone tests cannot give accurate results while you are taking birth control pills, so speak to our doctor about stopping them at least 2 months before your test
  • Time of day – the testosterone test and the Female Acne / Weight Gain Screen need to be done between 8.30am and 10am for accurate results
  • Menstrual cycle – the Female Acne / Weight Gain Screen is recommended on days 2 to 5 of your menstrual cycle
  • Fasting – the Men & Women Wellness screening needs 8 hours of fasting before your blood test

Always follow the preparation instructions our clinic gives you when you book your test.
