What Is Japanese Encephalitis?

Japanese encephalitis (JE) is a viral infection transmitted through mosquito bites that can lead to inflammation of the brain.

It is caused by a flavivirus and is endemic in various parts of Asia, including Southeast Asia, with outbreaks also reported in other regions.

Japanese Encephalitis Symptoms

In the majority of cases, individuals infected with JE do not exhibit symptoms. When symptoms develop, they can be severe and may include fever, headaches, nausea, seizures, and confusion.

These signs typically emerge in about 1 in 250 infected individuals. Around 30% to 50% of those who survive may suffer from long-term neurological issues such as vision or hearing impairment, speech difficulties, memory loss, seizures, or weakness in the limbs.

In more serious cases, the illness may progress to coma or even result in death.

Japanese Encephalitis Prevention

Minimising exposure to mosquitoes is key to preventing JE. Some practical tips include:

Wearing long-sleeved, permethrin-treated clothing
Applying insect repellent to exposed skin
Using mosquito nets, particularly during evening and nighttime hours

Who Is the Japanese Encephalitis Vaccine Recommended for?

Vaccination is generally advised for the following groups:

Residents of Endemic Regions – Individuals planning to reside in areas where JE is common.
Long-Term Travellers – People staying for a month or longer in regions where JE is prevalent, or those with repeated exposure.
Short-Term Travellers with Elevated Risk – Individuals visiting for less than a month but with risk factors such as rural travel, outdoor activities, or peak transmission season.
Travellers with Uncertain Plans – Those visiting areas where JE is endemic but have unclear travel routes, durations, or activities.

Who Should NOT Receive the Vaccine?

The JE vaccine may not be suitable for:

Individuals with Severe Allergies – Those who have had serious allergic reactions to previous JE vaccines (e.g. Ixiaro^®) or to any vaccine components, including protamine sulphate.
Pregnant Individuals – As a general precaution, JE vaccination is not routinely recommended during pregnancy unless the benefits clearly outweigh potential risks, such as when travel to high-risk areas is unavoidable.

The vaccine is administered via an intramuscular injection, typically into the upper arm.

For individuals who have not previously been vaccinated, a single dose of the Imojev vaccine is generally sufficient.

It is recommended to receive the vaccine at least 30 days before travelling to a region where Japanese encephalitis is endemic.

How Long Does the Japanese Encephalitis Vaccine Last?

One dose of the Imojev vaccine generally provides long-term protection that lasts for more than 10 years.

However, if there is ongoing risk beyond one year, your doctor may recommend a booster dose.
