What Is an STD Test?

An STD test is used to identify infections that are primarily spread through sexual contact. Sexually transmitted infections (STIs) can include chlamydia, gonorrhoea, syphilis, genital herpes, trichomoniasis, hepatitis B, and HIV.

There is no universal test that screens for all STDs or STIs. The selection of tests is often based on your symptoms, risk profile, and sexual history. Most screening procedures are relatively quick, using urine, blood, or swab samples.

Testing for STDs is crucial in safeguarding both your health and that of your sexual partners. Many infections can be asymptomatic, meaning you may not realise you are infected without a medical test. Identifying an infection early enables timely treatment and lowers the risk of complications.

Routine testing is especially recommended for those with new or multiple partners, or if there has been potential exposure. Even in the absence of symptoms, regular screening supports responsible sexual health management and helps prevent onward transmission of infections.

Common STDs such as chlamydia and gonorrhoea can be detected through a simple urine or swab test. If left untreated, these infections may lead to fertility issues.

How to Prepare for an STD Test?

  1. Fasting is not required. You may eat and drink as usual before your appointment.
  2. If your test involves a urine sample:
    • Try to avoid urinating for one to two hours before your visit. This helps ensure that the sample is concentrated and suitable for accurate testing.
    • When collecting the sample, provide the first part of your urine stream. This is known as the first void and should go directly into the sterile container provided.
    • Do not clean your genital area before collection unless your doctor has specifically advised you to do so.

Can STD Tests Be Wrong?

While STD tests are generally reliable, no medical test is 100% perfect. Understanding test accuracy can help you interpret your results and know when additional testing may be needed.

Urine Test Accuracy

Urine tests for sexually transmitted disease screening (such as chlamydia and gonorrhoea) are generally accurate, but their ability to correctly identify infections (sensitivity) can vary. For women, sensitivity ranges from 44% to 100% and specificity (correctly identifying those without infection) is 96% to 100%. For men, sensitivity is 89% to 100% and specificity is typically above 99%.

Blood Test Accuracy

Blood tests are mainly used for infections like syphilis and HIV. For syphilis, sensitivity ranges from 61% to 100% and specificity from 94% to 100%, depending on the stage of infection and the type of blood test used. Modern laboratory-based HIV blood tests have sensitivity ranging from 93% to 100% and specificity from 99.91% to 100%, as demonstrated in recent clinical studies.

Swab Test Accuracy

Swab tests (vaginal, cervical, urethral, rectal, or pharyngeal) are highly accurate. The general range for the sensitivity and specificity of nucleic acid amplification tests for chlamydia is approximately 89% to 100% sensitivity and 95% to 100% specificity, and for gonorrhoea is approximately 90% to 100% sensitivity and 99% to 100% specificity across specimen types and patient populations.
