What Is HPV?

HPV, or Human Papillomavirus, is a common sexually transmitted infection (STIs) that most sexually active individuals will encounter at some stage in life. With over 100 different strains, HPV can affect the body in different ways depending on the type of virus involved.

HPV remains a key public health concern in Singapore because of its strong association with various cancers. Approximately 1 in 10 healthy women carry HPV, with the highest prevalence among women aged 20 to 24.

Among those who test positive for HPV, approximately half are found to carry high-risk strains that are strongly associated with the development of cervical cancer.

In 2020, genital warts, which are most commonly caused by low-risk HPV strains, were the second most commonly reported STI in the country.

STI New Cases per 100,000 People
Chlamydia 26.9
Genital Warts 15.3
Gonorrhoea 11.8
Syphilis 11.2
Genital Herpes 6.9

HPV vs HIV: What’s the Difference?

HPV and HIV are both STIs, but they differ in the types of cells they affect and their long-term impact on health. HPV targets the skin and mucous membranes, and depending on the strain, may increase the risk of certain cancers.

HIV affects the immune system by attacking CD4 cells, leading to progressive immune weakening and, if left untreated, the development of AIDS.

What Are Symptoms of HPV?

Most HPV infections, especially those caused by high-risk strains, do not produce noticeable symptoms. When symptoms do occur, they typically manifest as different types of warts, including genital warts, flat warts, or plantar warts.

How Is HPV Transmitted / Spread?

HPV is primarily transmitted through intimate skin-to-skin contact, including vaginal, anal, or oral sex with an infected person. It can also spread through direct contact with infected areas, such as genital warts, or contaminated surfaces, especially when the skin has cuts, sores, or other openings.

Factors that increase the risk of HPV transmission include:

What Is an HPV Test?

An HPV test checks for the presence of HPV in cells collected from the cervix. During the procedure, a speculum is gently inserted into the vagina to allow access to the cervix, and a soft brush is used to collect a sample of cervical cells.

Most women report little to no discomfort, and the test is usually quick. The collected sample is then sent to a laboratory to identify whether any high-risk HPV strains are present.

HPV Test vs. PAP Smear: What’s the Difference?

A PAP smear and an HPV test serve different but complementary purposes in cervical cancer screening. While the PAP smear focuses on identifying abnormal or precancerous changes in cervical cells, the HPV test specifically detects high-risk HPV strains that can cause those changes.

When used together, these tests offer a more thorough assessment of cervical health and support early detection and timely medical care.

How Often Should I Do an HPV Test?

In Singapore, women aged 30 and above who are sexually active are encouraged to take an HPV test every five years. In addition, a PAP smear is recommended every three years to monitor for early signs of cervical cell changes.

For a more proactive and thorough approach to screening, you may consider doing both tests together at suitable intervals. If you are uncertain about your screening schedule, speak with your doctor for personalised guidance.

Can HPV be Cured?

There is currently no definitive cure for HPV itself. However, in most cases, the body’s immune system clears the virus naturally within two years. During this time, regular testing is important to monitor the infection and ensure that no complications develop.

If complications do arise, treatment may involve the removal of genital warts, monitoring and treating abnormal cervical cell changes, or managing HPV-related cancers if they occur.

How to Prevent HPV?

HPV prevention involves vaccination, safe sexual practices, and regular screening to reduce the risk of infection and help prevent HPV-related complications.

1. Vaccination

The most effective way to prevent HPV is through vaccination before any exposure to the virus. It protects against several strains of the virus, including those linked to cancer and genital warts.

Even if you have already been exposed to one strain, the vaccine can still offer protection against others and can help reduce the risk of complications.

2. Safe Sexual Practices

Using condoms or dental dams during sexual intercourse helps lower the risk of transmission by reducing skin-to-skin contact. While not fully protective, they can significantly reduce exposure to the virus.

3. Regular Screening

Routine HPV testing allows for early detection of high-risk infections. If abnormal cervical cell changes are found, they can be monitored or treated before progressing to more serious conditions.
